What You’ll Need

• NYS RN license required
• Associate’s Degree in Nursing (experienced candidates) or BSN (new graduates)
• 1 year of RN experience in managed care, home care, acute care, or nursing home preferred
• Certified Case Manager (CCM) preferred
• Bilingual proficiency required

What You’ll Do

• Provide case management for members, addressing medical, social, psychological, physical, and spiritual needs
• Develop, implement, and monitor care plans in collaboration with PCPs, caregivers, and the care team
• Ensure high-quality, personalized care for all members
• Support the goals and values of the managed care health plans
