Job Description - RN Care Manager/CDE

Description

Exciting opportunity to join a fast-paced medical team within a growing federally qualified community health center. HealthFirst Family Care Center, Inc. “The Heart of Our Community” provides comprehensive services including adult/pediatric medical care, health education, health assessment and screenings, gynecology, cardiology, behavioral health, substance use treatment, and adult/pediatric dental care.

RN Care Manager/CDE Summary:

Care Management is the component that links patients with broader healthcare and social support systems that ensure timely follow-up. Under the general supervision of the Clinical Director, the RN Care Manager performs the primary role functions of assessment, planning, facilitation and advocacy that is achieved through collaboration with the patient’s care team.

Schedule:

Mon-Fri 8:00 am – 4:30 pm

Required Qualifications:

  1. License to practice as an RN from the Commonwealth of Massachusetts and BA/BS degree or higher in health and human services.
  2. Certified Diabetes Educator (CDE) or Certified Diabetes Outpatient Educator (CDOE) required.
  3. Minimum two years’ experience with direct nursing care or community setting.
  4. Case Management experience along the care continuum addressing comprehensive care, involving all parties, and including client contact.
  5. Knowledge of health, social service and funding sources.
  6. Appropriate continuing education.
  7. Maintain current certification in CPR at the healthcare provider level.
  8. Strong computer knowledge.
  9. Bi-lingual skills preferred.
  10. Must submit a CORI Release Form and demonstrate proof of being free from disqualifying information.

  2. Strong computer skills required to complete online applications.
  3. Must possess excellent oral and written communication skills to effectively communicate with patients and other agencies.
  4. Ability to handle confidential information in a professional manner and in accordance with HIPAA guidelines.
