RN Case Manager

Looking for flexibility? We’re hiring full-time and part-time RNs to provide in-home care. Whether you want a full caseload or a lighter schedule, we’d love to hear from you.

• Assess patients’ physical, emotional, and social needs.


• Develop and update care plans with patients and caregivers.


• Provide direct nursing care, including wound care, infusion therapy, and medication management.


• Educate patients and caregivers to help them regain independence.


• Monitor patient progress and communicate updates to medical providers.


• Supervise and guide LPNs and HHAs in patient care.


• Keep accurate, timely records of care plans, progress, and treatment.


• Participate in on-call, weekend, and holiday rotations as needed.


• Stay current with nursing best practices through ongoing education.

• Active NY State RN license.


• At least 1 year of recent geriatric care experience.


• Home care experience is a plus.
