RN Case Manager

Description

This role involves working closely with patients, families, healthcare providers, and community resources to ensure optimal patient outcomes and quality of care - providing comprehensive care coordination and management for patients with mental health conditions..

JOB DUTIES & RESPONSIBILITIES

1. Assess and evaluate patients' mental health needs, developing individualized care plans.
2. Coordinate and monitor patient care across various healthcare settings and providers.
3. Collaborate with interdisciplinary teams, including psychiatrists, psychologists, and social workers.
4. Provide patient and family education on mental health conditions, treatments, and coping strategies.
5. Manage medication regimens and monitor for effectiveness and side effects.
6. Conduct regular follow-ups with patients to ensure adherence to treatment plans.
7. Facilitate smooth transitions between inpatient, outpatient, and community-based care.
8. Advocate for patients' needs and rights within the healthcare system and community.
9. Maintain accurate and up-to-date patient records and documentation.
10. Identify and connect patients with appropriate community resources and support services.
11. Participate in crisis intervention and management as needed.
12. Stay current with best practices and evidence-based treatments in mental health care.

Attendance to one’s entire shift is an essential function for the role.

Requirements

POSITION QUALIFICATIONS

Education Associate Degree in Nursing – required

Bachelor Degree in Nursing – BSN – preferred

Experience

• 1 year of medical rehab and/or case management experience a plus
• Strong knowledge of mental health disorders, treatments, and medications
• Excellent communication and interpersonal skills
• Proficiency in electronic health records (EHR) systems
• Ability to work independently and as part of a multidisciplinary team
• Demonstrated problem-solving and critical thinking skills
• Compassionate and patient-centered approach to care

Certifications Current RN License in good standing in the State of Practice

Case management certification (e.g., CCM, CMAC) preferred
