is a growing hospice care agency providing quality services to patients in the comfort of their homes. We are seeking a
Field Registered Nurse Case Manager to plan, organize, and direct hospice care in the Riverside area. 

The incumbent should be experienced in nursing with an emphasis on hospice and palliative care.

• Corona


• Eastvale


• Perris

• Grand Terrace


• Norco


• Jurupa Valley


• Moreno Valley

Registered Nurse Case Manager Responsibilities:

• Performs initial client assessments.


• Oversees admissions, re-certifications, and discharges of our home care clients.


• Coordinates the appropriate care plan for clients.


• Works collaboratively with other healthcare disciplines.


• Administers medications and treatments as prescribed by the physicians in the physician's plan of care.


• Work collaboratively with other healthcare disciplines


• On-call on a rotation

Registered Nurse Case Manager Requirements:

• Licensed as a Registered Nurse/RN


• Valid Driver's License and reliable vehicle with current insurance.


• Experience with Hospice MD.


• 1 yr of Hospice experience.


• Demonstrated ability to work autonomously.


• Excellent at planning, organizing, and communicating.
