RN Case Manager

We are hiring a Full-time Registered Nurse (RN) to serve as Case Manager for seriously ill patients.

 As a case manager, you will provide skilled nursing services in a patient’s residence, which includes assessing, planning, implementing, and evaluating phases of nursing care. You will work alongside your clinical and operational team members to ensure the provision of high-quality hospice services. Candidates must possess excellent assessment skills, critical thinking skills, and strong organizational abilities. Candidates should be energetic, have strong communication and interpersonal skills, and have a demonstrated ability to work independently and exercise sound clinical judgment.

Applicants must have

• 1-2 years of acute care experience; hospice or home care experience is strongly preferred. 


• Must have current NCBON licensing without restriction.
