RN - Case Manager

Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The ideal candidate will play a crucial role in coordinating patient care, ensuring that patients receive the appropriate medical services and support throughout their healthcare journey. This position requires a strong background in patient assessment and care management, particularly for patients with complex medical needs, including geriatrics and behavioral health.

Responsibilities

• Conduct comprehensive patient assessments to develop individualized care plans.
• Coordinate primary care services and ensure continuity of care across various healthcare settings.
• Maintain accurate medical records and documentation for all patient interactions.
• Perform diagnostic evaluations to identify patient needs and recommend appropriate interventions.
• Engage in utilization management to optimize resource allocation and improve patient outcomes.
• Collaborate with interdisciplinary teams to facilitate effective communication and care delivery.
• Provide education and support to patients and their families regarding treatment options and health management strategies.
• Monitor patient progress and adjust care plans as necessary, particularly for those in ICU or requiring specialized attention.

Years of Experience: 2 year(s)

Certifications: BLS, CA RN License

Charting System: Epic, Interqual experience are required.

Experience

• Current Registered Nurse (RN) license in good standing.
• Proven experience in patient care, particularly within primary care settings or case management roles.
• Familiarity with medical records management and documentation practices.
• Experience working with geriatric populations and understanding their unique healthcare needs.
• Knowledge of behavioral health issues and the ability to conduct appropriate assessments.
• Strong skills in diagnostic evaluation, critical thinking, and problem-solving.
• Previous experience in utilization management is preferred but not required.

Shift: Day 5x8-Hour (08:00 - 16:30)

Pay: $60.84 per hour
