Position Overview

We are seeking a skilled Registered Nurse Case Manager to join our team. This in-office position, based in Aiken, SC, requires availability Monday through Friday, with additional travel to patients' homes as needed. This role is crucial in assessing patient needs, coordinating care, and ensuring effective communication among all parties involved in patient care.
