  (Context: Glen Carbon is ~19 miles NE of downtown St. Louis—about a 25–30 minute drive. We prioritize local, community-based assignments with shorter commutes.)

  Lead patient- and family-centered hospice care across your local territory.

  Complete comprehensive assessments; create and update individualized plans of care.

  
  


• 
  

  Educate families on symptom management, medications, and what to expect—at a humane pace.

  
  


• 
  

  Collaborate with physicians, social workers, chaplains, aides, and volunteers.

  
  


• 
  

  Document accurately and on time in our EMR.

  
  


• 
  

  Participate in IDT meetings and advocate for what your patients—and you—need.

  
  

• 
  

  License: Current, unrestricted Illinois RN.

  
  


• 
  

  Experience: 1–2+ years as an RN; hospice strongly preferred (palliative/geriatric/home health valued).

  
  


• 
  

  Strong assessment and communication skills; ability to work independently with a supportive team.

  
  


• 
  

  Valid driver’s license, reliable transportation; EMR proficiency (NetSmart a plus).

  Pay: $45.00/hour flat. (No sign-on bonus—we invest in your day-to-day support and growth.)

  Schedule: Full-time with flexibility aligned to patient needs; optional dedicated on-call roles available.
