Job Summary:

As an RN Case Manager at Telenett, you will be responsible for coordinating and managing the care of patients who require ongoing medical treatment. You will work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the best possible care and support. This is a full-time position, with a schedule of 40 hours per week.

Key Responsibilities:

- Conduct initial assessments of patients' medical needs and develop individualized care plans

- Collaborate with healthcare team members to ensure that care plans are implemented effectively and efficiently

- Monitor patients' progress and make necessary adjustments to care plans

- Educate patients and their families on their medical conditions and treatment plans

- Coordinate appointments and referrals for patients with other healthcare providers

- Communicate with insurance companies to obtain necessary authorizations for medical treatments

- Document all patient interactions and maintain accurate records

- Participate in multidisciplinary team meetings to discuss patient care and progress

- Stay up-to-date on current healthcare practices and regulations

- Provide support and guidance to patients and their families during difficult medical situations

- Act as an advocate for patients to ensure they receive the best possible care

Qualifications:

- Registered Nurse (RN) license 

- Minimum of 2 years of experience in case management or a related field

- Excellent communication and interpersonal skills

- Strong organizational and time-management abilities

- Ability to work independently and as part of a team

- Proficiency in electronic medical records (EMR) systems

- Knowledge of healthcare regulations and best practices
