Registered Nurse (RN) Case Manager – Home Care
Full-Time | Field-Based | Fairfield County, CT
Pediatrics & Geriatrics | Concierge Home Care

Quick Overview

Schedule: Full-Time
Setting: Field-Based / Home Care
Patient Population: Pediatrics & Geriatrics
Territory: Fairfield, Litchfield, Hartford & New Haven Counties

What You’ll Do

Provide and oversee high-quality in-home nursing care
Develop and manage individualized care plans
Coordinate with physicians and interdisciplinary care teams
Monitor, document, and report changes in patient condition
Supervise and support home care staff
Ensure patient safety, comfort, and independence at home

What We’re Looking For

Active Connecticut RN license
Minimum 2+ years RN experience (acute care required; home care preferred)
Experience with pediatric and geriatric patients
Clinical skills in:
- Ventilator and tracheostomy care
- G-tube/PEG tube management
- IV infusions and wound care
Current BLS certification
Valid driver’s license
Authorized to work in the U.S.
