RN - CASE MANAGER 

Description:

Reporting to the Utilization Review Manager, the Case Manager is responsible for the coordination of the continuum of patient care in accordance with the vision and goals of Doctors Medical Center. The RN ensures quality, cost-effective patient care through planning, communication, critical thinking and facilitation of the multi-disciplinary patient care team.

Qualifications:

CA RN license
2+ years acute care experience required
1+ years Case Management experience in acute hospital required
Interqual experience required
Weekend & Holiday rotation required

2+ to 5 years experience
