RN Case Manager needed for Hospice Care in the Grand Rapids area!

Salary:

$80k - $85k (negotiable, based on hospice experience)

JOB SUMMARY:

The Registered Nurse will work as a member of the Hospice team provide nursing care to the terminally ill hospice patient as needed along with assistance and understanding to the family and in time of bereavement. This position will see about 12-18 patients weekly in a skilled nursing building.

QUALIFICATIONS:
Graduate from an accredited school of nursing.
Currently licensed as a Registered Nurse to practice in the state.

At least 1 year of Hospice nursing experience as a Registered Nurse,

REQUIRED .

Must have flexibility for rotating on-call shifts. on-call every third weekend
TRANSPORTATION:

Must have a current valid driver's license, auto liability insurance and reliable transportation.
Competitive mileage rates will be paid at $0.58 per mile.

Must make judgments based on assessments and data available and act accordingly.

Acceptance of and adaptability to different social, racial, cultural and religious modes.
Ability to complete Hospice training program.
