The Hospice Registered Nurse Case Manager ensures quality and safe delivery of hospice services. Plans, manages, coordinates and/or delivers services provided in the home and in facilities utilizing computerized documentation system and telehealth. Registered Nurse Case Manager will provide hospice patients of Emblem Hospice with comprehensive care while maintaining continuity of care with all disciplines involved. This position will reflect the agency mission and values at all times, while adhering to standards, rules, and regulations for hospice.
