Schedule: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM | 1 Weekend Per Month | 1 On-Call Shift Per Week

Compensation: $95,000 – $130,000 annually plus incentive visit opportunities and $6,000 in first-year bonuses

When one of our territories needs additional clinical support, we need an experienced nurse who can step in and take ownership.

That's the Float RN Case Manager.

Rather than permanently managing patients in one geographic area, you'll rotate throughout different portions of our service area based on patient census, staffing, and clinical needs.

You'll typically spend approximately three months supporting an area, giving you time to learn the territory, manage your patients, work alongside the local clinical team, and become a dependable resource.

Then, when the need changes, you'll take that experience somewhere new.

This isn't random floating from one end of the metro to another every morning.

How the Float Position Works

Your territory will change based on where additional Case Management support is needed.

Rather than changing every few days, assignments will generally last approximately three months at a time.

During each rotation, you'll:

  • Become familiar with that portion of our service area
  • Manage an assigned home health caseload
  • Build relationships with patients and caregivers
  • Work closely with the interdisciplinary team supporting that area
  • Provide continuity throughout your patients' episodes of care
  • Help stabilize coverage and ensure patients continue receiving timely care

When organizational needs shift, you'll transition to another territory and become part of that team.

New area. New patients. New challenges. Same standard of nursing.

When you're assigned a patient, you're responsible for understanding the whole clinical picture.

You'll assess where they are today.

Understand why they entered home health.

Develop and manage their plan of care.

Coordinate with physicians and therapists.

Educate patients and caregivers.

Recognize when something changes.

And help move that patient toward their goals.

Your ZIP code may eventually change.

Your responsibility to your patients doesn't.

What You'll Do

  • Manage an assigned home health caseload within your current coverage territory
  • Rotate throughout different portions of our service area based on clinical and staffing needs
  • Complete OASIS Start of Care, Recertification, and Discharge assessments
  • Develop and update individualized plans of care
  • Perform comprehensive nursing assessments and skilled visits
  • Provide wound care, medication management, patient education, and other skilled nursing services
  • Monitor patient progress and recognize changes in condition
  • Communicate with physicians regarding clinical concerns
  • Coordinate with PT, OT, SLP, social work, and other interdisciplinary team members
  • Help patients avoid unnecessary hospitalizations and remain safely at home
  • Maintain timely and accurate clinical documentation in Homecare Homebase (HCHB)

We're looking for:

  • Active Minnesota Registered Nurse (RN) license
  • Strong comprehensive assessment and clinical decision-making skills
  • Home health and OASIS experience strongly preferred
  • Confidence managing an independent patient caseload
  • Strong organization and time-management abilities
  • Excellent patient, caregiver, physician, and team communication
  • Flexibility to transition between territories as organizational needs change
  • Reliable transportation and willingness to travel throughout our service area
