Job Description - RN Case Manager Floating Territories

Job Title: Floating RN Case Manager – Home Health

Company: Legacy Home Care (Part of the Home Health Care, Inc. Family of Companies)

Compensation: $95,000 – $130,000 per year + Incentive Visits

Location: Greater Twin Cities Metro Area

Are you a skilled and adaptable nurse looking to take your home health career to the next level? At Legacy Home Care, part of the trusted Home Health Care, Inc. family of companies, we’re hiring a Floating RN Case Manager to provide support across our growing service areas in the Greater Twin Cities.

This full -time position is ideal for nurses who enjoy variety and are eager to jump in where they’re needed most—whether that’s covering fluctuating caseloads or assisting in another territory every few months. You'll still enjoy the consistency of 1:1 care in the home setting, with added flexibility and a strong support system behind you.

  • Incentive pay: Earn additional compensation for visits in support of fluctuating census or expanded territory needs

Key Responsibilities:

  • Deliver skilled nursing care and perform comprehensive client assessments
  • Develop and update individualized care plans
  • Provide caregiver education and support to maintain quality standards
  • Rotate into the on -call schedule
  • Travel as needed to different areas of the Twin Cities metro to assist with coverage

Qualifications:

  • Active RN license in the State of Minnesota
  • Reliable transportation and valid insurance (position involves regular travel)
  • Strong clinical judgment and communication skills
  • Experience in home health is preferred, but not required—we’re happy to train compassionate, motivated RNs
