• Provide direct nursing care in home settings
• Perform comprehensive health and psychosocial evaluations
• Monitor and educate patients on their conditions
• Coordinate with Physical Therapists, LPNs, OTs, and Social Workers
• Complete OASIS documentation (admission, transfer, recertification, etc.)
• Participate in case conferences and staff meetings

• RN license in good standing (VA)
• Three or more years of RN experience (home health preferred)
• Reliable transportation and valid driver’s license
• CPR certification
• Ability to pass background checks and provide references
