RN Case Manager - Home Health

Locations: San Diego, Orange County, Riverside

Schedule: Flexible, per diem

Compensation: $60–$120 per visit (average ~$65/hour)

We're hiring RNs to provide skilled nursing care in patients' homes. This is a per-visit role with real volume behind it — ideal for nurses who want autonomy, consistency, and the ability to earn without being tied to a facility schedule.

  Complete in-home skilled nursing visits

  Perform assessments, treatments, and care plan updates

  Educate patients and caregivers on care, medications, and safety

  Document visits accurately and on time

  Coordinate with the broader care team as needed

  Active California RN license

  
  


• 
  

  1+ year of nursing experience (home health preferred, not required)

  
  


• 
  

  Strong clinical skills and confidence working independently

  
  


• 
  

  Reliable transportation to travel to patient homes

  
  


• 
  

  Proficient with EMR documentation

  CPR/BLS certified

  
  

• 
  

  Consistent patient volume – We’re placing 20–30 RNs/month to meet demand

  High earning potential – RNs are averaging 5–6 visits/day, earning $500–$700/day
