Job Title: RN Case Manager – Independent Nursing with a Personal Touch

Location: Greater Twin Cities Metro Area

Schedule: Full-Time (M-F, 8:00 AM – 5:00 PM), 1 weekend/month, 1 to 2 on-call shifts per weeknight per month.

Compensation: $85,000 – $130,000 annually + $6,000 in performance-based bonuses

Daily Responsibilities:

• Deliver comprehensive nursing visits and care planning
  
• Monitor patient outcomes and adjust interventions as needed
  
• Coordinate services with physicians, therapists, and care aides
  
• Lead family education and health management guidance
  

Requirements:

• RN license in MN
  
• 3 years RN experience (home health preferred)
  
• Reliable car and driver’s license
