Job Description - RN Case Manager - Professional Development Opportunities

Schedule:

  • Our RN Case Managers provide support from 8 a.m. to 5 p.m., rotating weekends, and the 3rd weekend monthly.

 

An Excellent Opportunity

We have openings for RN Case Managers (Home Care) in Martinsville, VA. service area. The RN Case Manager provides nursing care to patients with a life-limiting diagnosis, consistent with a physician’s order, nursing assessments and individualized plans of care.

Mountain Valley provides continuous (24/7) patient care. The work schedule may include day or evening shifts, weekends, holidays, and extended schedules. In extenuating circumstances, staff may be asked to work more than their scheduled hours in a day/week

Responsibilities:

  • Meet Our Mission of "Creating the best experience for those facing serious illness... Every Patient, Every Family, Every Time".
  • Conduct an individualized patient assessment, prioritizing the data collection based on the patient’s immediate condition or needs and prioritizing care accordingly.
  • Manage the plan of care by assessing, implementing, and evaluating the services provided to each patient.
  • Provide support and individualized patient/family education customized to the patient and their family.
  • Provide ongoing assessments of patients, recording significant conditions and reactions, and reporting conditions to appropriate team members.
  • Collaborate with members of the interdisciplinary group to assure that the aims of continuity and comprehensiveness of care are accomplished.
  • Prepare clinical and progress notes for the electronic medical record, and ensure all documentation is current and consistent with established standards.
  • Participates in providing bereavement support to caregivers, as requested.
  • Provide direction to licensed practical nurses and certified nursing assistants, in the ongoing implementation of the care plan.
  • Supervisory duties of LPNs and CNAs include visits to the patient’s home at least every 14 days, patient record reviews, conferences, ongoing communication, and collaborative care planning.
  • Attend staff development programs as required by policy and maintain current competencies.
  • Provide service in a non-judgmental, non-discriminatory manner that is sensitive to the employee's, patient's, and family's diversity, preserving their autonomy, dignity, and rights.
  • Maintain the highest standards of professional confidentiality in matters related to the patient, family, and facility staff.
  • Perform other assignments as appropriate.

Qualifications:

  • Graduation from an accredited nursing program.
  • Current, unrestricted registered nursing license to practice nursing in North Carolina and/or Virginia.
  • Must maintain CPR certification.
  • Valid Driver’s License and current automobile insurance.
  • 12 hours of continuing education annually.
  • Work various shifts that may include days, nights, weekends, and holidays.
