RN - Case Manager - Travel - Days - Wildomar, CA

24-Hour Medical Staffing Services, LLC is hiring an experienced Case Manager Registered Nurse (RN) to join our growing team of healthcare professionals.

Job Overview:

• Job Title: Case Manager Registered Nurse (RN)
• Location: Wildomar, California
• Type of Assignment: Contract (13 weeks)
• Start Date: ASAP
• Shift:
  • 5x8 Hours, 40 hours per week
• • Weekend Requirements: Yes, to meet department needs
• Shift Hours: 08:00 - 16:30
• Pay Rate: $3400 - $3450 per week

Qualifications:

• Valid California License
• BLS Certification (must be through American Heart Association - AHA)
• Minimum 2-year recent experience as a Care Coordination, Discharge Planning and Utilization Review in an acute care hospital
• Must have travel experience
• Experience with HMOs, IPAs, etc.
• Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc.
• Competent with arranging DME, SNF and ambulance transport.
