Job Title: Registered Nurse (RN) Case Manager – Care at Home Expert

Company: Home Health Care, Inc.

Location: Greater Twin Cities Metro Area

Schedule: Full-Time (Monday to Friday, 8:00 AM – 5:00 PM), 1 weekend/month, 1 to 2 on-call shifts per month

Compensation: $85,000 – $130,000 annually + $6,000 in bonuses during first year of employment

Key Responsibilities:

• Conduct initial assessments, develop personalized care plans, and complete OASIS timepoints
  
• Monitor clinical progress, coordinate care transitions, and manage interdisciplinary communication
  
• Educate and counsel patients and families about their conditions and care
  
• Ensure documentation meets state, federal, and agency standards
  

Qualifications:

• Active Minnesota RN license
  
• 3+ years nursing experience (home care preferred)
  
• Knowledge of home care compliance and strong assessment/documentation skills
  
• Valid driver’s license and insured vehicle
