Join our Assisted Living team as an RN Case Manager and coordinate personalized, high-quality nursing care that supports residents' well-being while collaborating with physicians, families, and the interdisciplinary care team.



Job Summary:


The RN Case Manager provides skilled nursing care to Assisted Living residents in accordance with physician orders and individualized care plans. As a valued member of the Assisted Living Care Team, this role coordinates care, promotes resident well-being, and delivers compassionate, person-centered nursing services.



Qualifications & Education: 



  • Graduate of an accredited school of nursing with a current Minnesota RN license in good standing.

  • CPR certification required.

  • Minimum of one year of nursing experience preferred, ideally in Assisted Living, senior living, home health, or long-term care.

  • Strong clinical assessment, critical thinking, and sound judgment skills.

  • Excellent written, verbal, and interpersonal communication skills.

  • Ability to recognize changes in resident conditions and respond appropriately.

  • Compassionate, patient-centered approach with a commitment to providing high-quality care and promoting resident safety, comfort, and independence.

  • Ability to collaborate effectively with residents, families, physicians, and the interdisciplinary care team.

  • Demonstrated leadership and organizational skills with the ability to work independently and supervise others when needed.

  • Ability to meet the physical requirements of the position and maintain required licensure and continuing education.

Key Responsibilities:



  • Coordinate and manage individualized resident care plans in collaboration with physicians, residents, families, and the interdisciplinary care team.

  • Provide skilled nursing services, including assessments, treatments, medication management, specimen collection, and resident education.

  • Monitor resident health, identify changes in condition, and communicate updates to physicians and appropriate team members.

  • Complete required clinical documentation, care plans, physician orders, and regulatory documentation accurately and timely.

  • Conduct routine resident visits, assessments, follow-up care, and emergency response as needed.

  • Supervise and support nursing staff and Home Health Aides through education, guidance, and collaboration.

  • Coordinate care with pharmacies, hospitals, laboratories, and other healthcare providers to ensure continuity of care.

  • Maintain knowledge of Assisted Living regulations, Medicare/Medicaid requirements, and organizational policies and procedures.

  • Participate in the on-call rotation, staff meetings, committees, and continuing education to support quality resident care.

  • Foster positive relationships with residents, families, coworkers, and community partners.

  • Perform other duties as assigned.
