Role Responsibilities:

• Conduct in-home skilled nursing visits and patient assessments
  
• Carry out physician-ordered treatments including wound care, IV therapy, and medication management
  
• Provide patient and caregiver education focused on safety, disease management, and wellness
  
• Maintain accurate and timely documentation in the EMR system
  
• Collaborate with interdisciplinary care teams and community resources
  

Qualifications:

• Active MN RN license and current CPR certification
  
• Valid driver's license, vehicle, and auto insurance
  
• 1+ year of clinical nursing experience; home health experience preferred
  
• Strong organizational and time management skills
  
• Comfort with independent fieldwork and electronic documentation
