Summary:
This position provides whole person health promotion and disease prevention services to a designated Evertrue Affordable Housing (AH) community(s) and identified surrounding partner (non-Evertrue) Affordable Housing locations.
Essential Duties and Responsibilities:
Empowers clients through education to manage their medical needs and to promote healthy aging
Provides individual and group health education and counseling related to health maintenance, disease prevention, or illness patterns
Encourages community residents to self-monitor their health
Provides health assessment and education, encouraging healthy lifestyle practices
Documents client assessment, nursing diagnosis, interventions and outcomes while maintaining confidential client record in accordance with the policy on documentation
May visit clients as needed providing follow-up health counseling, education, and emotional presence/support
Promotes stewardship of the body emphasizing self-care of the whole person
Collaborates with the Affordable Housing (AH) Service Coordinators to develop schedules for educational events, support groups, assessment appointments and overall health promotion activities in the AH community
Teaches and models the integration of faith and health into daily life
Works in conjunction with identified partners to provide educational programs based on identified needs for healthier lifestyles, early illness detection and health resources
Develops and facilitates support groups based on identified needs and resources and/or clients to participation in designated community groups
May act as a preceptor to students from schools of nursing, seminaries, and other disciplines as requested.
Participates in research related to the field of community nursing
Qualifications, Knowledge, Skills, and Abilities:
LPN or RN license in Missouri and/or Illinois required (BSN preferred)
Must be CPR BLS (basic life support) certified with the American Red Cross
5 years of clinical nursing experience preferred
Competent in nursing assessment skills
Community health nursing experience desirable
Able to provide health counseling
Works well independently and yet can function well as a part of a team
Possesses a good understanding of the relationship of spirituality and health

Evertrue Pay & Benefits
Pay grade: 14
Pay range: $31.05/hour to $49.75/hour
