RN Home Care Assessment

The Nurse Connection Staffing is looking for fee-for-service *Home Care Field RN* in the Syracuse and surround areas throughout Central New York to provide high-quality patient care in the home. The RN must travel to the patient’s home to administer healthcare services and complete the required assessments.

*Duties and Responsibilities:*
* Conduct ongoing patient care and assessments.
* Initiates Plan-of-Care and makes necessary changes as necessary.
* Prepares written documentation and summaries of the patient’s status and progress as needed to continue home health services.
* Maintain close communication with the Director of Patient Services and the interdisciplinary team.
* Responds to phone calls and emails in a timely manner.
* Reporting all issues and concerns to the Director of Patient Services immediately.
* Completes documentation accurately and submits timely to the agency
* Provides the highest quality care in accordance with DOH regulations.
* Complete evaluation tasks, including reviewing medication and vital signs.
* Administer Physician-prescribed medication.
* Dress or redress wounds and assess the healing progress.
* Provide education to patients and families on proper home health care procedures and strategies.

*Qualifications*
* Graduate of an approved school of nursing
* Current NYS RN license
* Liability insurance is required.
* Excellent communication skills, detail-oriented, patient care, and assessment skills
* Able to function independently with minimum supervision.
* Demonstrates strong critical thinking skills.
* Ability to travel to patients’ homes.
* Long-term care 1-year of experience preferred.
* Nursing home care 1-year of experience preferred.
* Rehabilitation center 1-year of experience preferred.

Pay: $45.00 - $50.00 per hour
