RN Home Care Assessment Nurse (PRN)

Eminence Home Care is seeking a dedicated and compassionate PRN RN Assessment Nurse to join our dynamic healthcare team. Responsibilities

• Conduct initial and ongoing patient assessments to evaluate health status and care needs.
• Develop individualized care plans in collaboration with healthcare team members and patients.
• Communicate assessment findings effectively to ensure continuity of care.
• Monitor patient progress and modify care plans as necessary based on clinical evaluations.
• Educate patients and their families about care procedures, medication management, and health maintenance.
• Provide guidance and support to caregivers and families to enhance patient care.
• Document all assessments, interventions, and outcomes in compliance with regulatory standards.

• Current RN license in the state of NH
• Bachelor's degree in nursing (BSN) preferred.
• Minimum of 2 years' nursing experience, preferably in home health or community settings.
• Drivers License and vehicle
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills for effective patient interaction.
• Ability to work independently and manage time effectively.
• Current CPR and Basic Life Support (BLS) certification.
