Home Health RN - Santa Clarita CA / Surrounding areas
- Start Date: ASAP
- Shift: 8hr Days (*This facility cannot accommodate 4x10s or 3x12s.*)
- Ratios: 1:1 (*May visit up to 6 patients per day; 30 productivity points standard)
- Years of Experience REQ: 1 YR of Skilled Home Health Experience
- First-Timers Accepted: No
- Weekend REQ: 1 Weekend Shift Per Contract
- Certs REQ: BCLS
- Is On-Call REQ: No
- Will this traveler need to float between like units: No
- Open to accommodating block schedule: No
- Locals Accepted: Yes; Considered "local" within 50 mile radius, will accept locals at the same rate
- Pending License Accepted: Yes
- RTO Restrictions: Confirm with hiring manager
- Guaranteed Hours: Facility can call off UP TO one shift per pay period (every two weeks)
Special Requirements
- Traveler is expected to have their own transportation.
- System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles.
- Oasis is required.
Please call out if traveler does not have Oasis ExperienceAs a Home Health RN, responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families. Patient care is delivered within organizational philosophy, policy and standards of community health practice. Also required to perform admission processes and case management responsibilities in accordance with policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
ALL APPLICANTS MUST HAVE 1 year Traveler and 1 year Home Care experience. 1 year Case Management Experience. Ability to travel to cases as assigned,
- Must have a current and valid State license as a Registered Nurse
- BLS upon hire and maintain current.
- Working knowledge of Mental Health Laws and practice standards.
- Working knowledge of interdisciplinary treatment team process, behavioral health programming and daily nursing functions for inpatient and outpatient behavioral health units.
- Working knowledge of the continuum of care for behavioral health care.
- Behavioral Violence Prevention within 6 months of hire and maintain current annually
