RESPONSIBILITIES:
Provide nursing services to patients in their homes in accordance with regulations, policies, and accepted professional standards of practice. Responsibilities of the Home Health Nurse include patient assessments, coordination of care, case management, documentation, and communications between all members of the care team.

QALIFICATIONS:

Current Connecticut RN license

Valid driver’s license

Reliable transportation and willing to travel in Windham County

Strong communication and interpersonal skills

Home health experience preferred
