Summary

Are you a nurse looking for an office setting where you can still pursue your love of providing care and helping others do the same? The Registered Nurse Case Manager works with constant support alongside our skilled staff out of our well-established BrightStar of Fort Wayne office.

As our RN Case Manager, you would oversee the care provided to our clients as directed by BrightStar Care and our Joint Commission accreditation quality and safety guidelines.

What You'll Do:

• Supervision of clinical field staff to provide top-notch care for our clients


• Perform new client assessments


• Oversee the care of existing clients


• Medication reconciliation


• Care coordination with skilled service organizations/hospices


• Clinical on-call rotation

What We'll Love About You:

• Current unrestricted RN license in the state of Indiana


• Certified CPR and negative TB skin test or chest x-ray


• Minimum of one (1) year of documented nursing experience required. Home health experience preferred


• Licensed driver with valid automobile insurance


• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds
