What You’ll Do

• Be available (in person, by phone, or electronically) 24/7 during operating hours as required by regulations
• Oversee patient and staff assignments
• Coordinate patient care and manage referrals with continuous assessment
• Review quality assurance documentation for assigned records
• Manage and monitor patient and clinician schedules
• Stay current with OASIS data set requirements, home care reimbursement, and compliance
• Develop expertise in agency software systems
• Attend mandatory meetings and trainings as required
• Actively participate in quality assurance and performance improvement initiatives
• Conduct home visits and complete comprehensive/OASIS assessments as needed
• Serve as the liaison between clients, physicians, and Phoenix Home Care
• Participate in the on-call rotation as assigned
