Schedule: 7pm-7am



Position Summary:


The Partners In Hope Foundation in collaboration with Partners In Home Care is hiring for a Hospice RN at its Hope Hospice Center that will provide skilled, compassionate care to patients in an inpatient hospice setting. This role ensures comfort, dignity, and quality of life for terminally ill patients, while also supervising and directing the Certified Nursing Assistants (CNAs). The RN works closely with interdisciplinary teams to develop and implement care plans that reflect the patient’s needs and wishes. This role is a Nights 7PM-7AM, 1.0 FTE.



Essential Responsibilities:



  • Patient Care & Symptom Management:
    Provide comprehensive nursing care to terminally ill patients, ensuring symptom management and comfort through medication administration and interventions as per physician orders. Assess and respond to patient needs, ensuring a dignified and comfortable end-of-life experience.

  • Supervision & Delegation to CNAs:
    Supervise and guide non-unionized CNAs, ensuring they provide appropriate personal care and support per the patient’s care plan, facility standards, and their job description.


    • Delegation of Tasks: Appropriately delegate nursing support tasks, such as assisting with activities of daily living (ADLs), patient hygiene, and mobility assistance, while ensuring safe, compassionate care.

    • Mentorship & Education: Offer real-time guidance and mentorship to CNAs, providing feedback on performance while maintaining a respectful work environment.

    • Ensuring Proper Documentation: Monitor CNA documentation of patient care to ensure it is accurate and complete.



  • Interdisciplinary Team Collaboration:
    Participate in interdisciplinary team (IDT) rounds, contributing to patient care plans, symptom management, and family education. Collaborate with other healthcare professionals to ensure holistic patient care.

  • Documentation & Compliance:
    Accurately document nursing assessments, interventions, and communications with CNAs, patients, and families. Ensure all care provided complies with regulatory and quality standards.

  • Family and Caregiver Support:
    Provide emotional support, education, and guidance to patients' families and caregivers, assisting them in navigating the emotional and physical challenges of end-of-life care.

  • Infection Control & Safety:
    Adhere to infection control procedures and safety protocols to ensure a safe, comfortable environment for both patients and staff.



Qualifications:


Required:



  • Current compact or RN license in MT

  • Minimum of one year of recent hospice, palliative care, critical care, or med-surg nursing experience

  • Knowledge of and experience in a unionized healthcare environment (for RNs)

  • Strong clinical judgment and communication skills

  • Basic Life Support (BLS) certification


Preferred:



  • Hospice and Palliative Care certification (CHPN)

  • Previous experience in an inpatient hospice setting

  • Familiarity with electronic health records (EHR), Home Care Home Base a plus
