RN - Hospice Case Manager

Hospice & Palliative Care, Inc. is seeking a compassionate and experienced  Registered Nurse (RN) Case Manager to provide high-quality, patient-centered hospice care. The RN Case Manager plays a vital role in ensuring comfort, dignity, and quality of life for patients with terminal illness while offering guidance and support to their families. This position allows nurses to focus on meaningful 1:1 care in a variety of settings, including private homes, residential care facilities, nursing centers, and other hospice environments.

Schedule: Monday–Friday, 8:00 AM–4:00 PM
Status: Full-Time
Reports To: Clinical Supervisor

 Key Responsibilities

• Develop, implement, and evaluate individualized plans of care
• Monitor patient conditions and manage pain and symptoms through appropriate medication adjustments
• Provide education, emotional support, and guidance to patients and families
• Coordinate durable medical equipment (DME), medications, and specialty consults
• Collaborate with the Interdisciplinary Group (IDG) and other healthcare professionals
• Participate in Morning Report and bi-weekly IDG meetings
• Respond effectively to patient and family needs, including symptom management and crisis intervention

Qualifications

• Current New York State Registered Nurse (RN) license
• Minimum of 2 years’ experience as a Registered Professional Nurse Preferred 
• Strong clinical skills in bedside care, symptom management, crisis intervention, and family intervention
• Ability to work independently while collaborating with a multidisciplinary team
• Excellent communication and organizational skills
