Description

VNA Hospice of NWI is seeking:

Full-time RN for 14 Bed Inpatient Hospice Facility

3:00 p.m.-11:30 p.m. with every other weekend rotation


Position Summary:

The Registered Professional Nurse serves as a case coordinator by providing, arranging for, teaching and supervising the health care of hospice patients at the hospice center. The nurse appraises patient and family care needs and coordinates services with the interdisciplinary team.

  • Major Functions:
  • Assumes primary responsibility for nursing care of the patient in conjunction with physician’s orders and IDT.
  • Understands and can interpret to patient and family the philosophy, acceptance, admission, recertification, and discharge policies for all agency programs and levels of care.
  • Observes, evaluates, and reports to physician patient’s physical and emotional condition, reaction to drugs and/or treatments. Interprets to physician social and physical factors in the environment that affect patient’s care.
  • Identifies nursing problems and establishes plan of care based on mutual goals.
  • Makes ongoing re-assessment of patient/family’s current status and initiates changes in plan of care as necessary.
  • Evaluates need for additional agency and/or community resources and makes appropriate referrals including caregivers at risk for pathological grief.
  • Coordinates and/or implements other agency professional services for his/her patients and families. Prioritizes needs with the members of IDT.
  • Instructs and supervises HHA/LPN’s.
  • Consults with and educates the patient and family regarding the disease process, self-care techniques, prevention strategies, end of life care and process to deal with issues of ethical concern.
  • Helps patient and family to adjust positively to physical, mental and social limitations. Helps family accept and cope with responsibilities for providing care.
  • Respects the right of the patients to maintain reasonable control of their lives and retain as much decision-making authority as possible.
  • Helps to achieve and maintain continuity of patient care by planning and exchanging information with other health and social agencies as well as by intra-agency exchange of information.
  • Utilizes the team approach to achieve and maintain continuity and comprehensiveness of patient/family care.
  • Serves as an auxiliary member of IDT and attends IDT meetings when requested to do so. Participates in re-certification process per agency policy.
  • Abides by shift schedule. Performs other duties as assigned.
  • Provides specialized hospice training to other staff, family members, and informal caregivers to insure adequate care.

Requirements

 

Qualifications:

  • Must be currently licensed as a registered professional nurse by the state of Indiana.
  • Diploma must be from an approved school of professional nursing. Must maintain current CPR certification.
  • Minimum of one year of medical/surgical experience in a hospital setting, nursing home, ECF, or home care. Previous hospice experience and/or demonstrates understanding and support of the hospice philosophy and concept preferred. Demonstrates comprehensive knowledge and skill in current nursing practice.
  • Must hold and maintain a valid driver’s license, auto insurance, and professional liability.
  • Exhibits knowledge of the concept of a interdisciplinary team and a working knowledge of the development of plan for patient care and the ability to participate in them as well as using the case management approach.
  • Functions independently and appropriately.
  • Ability to use and adapt to current technology, i.e. computers, laptops, pagers, infusion pumps.

  • Acute Care: 1 year (Preferred)

License/Certification:

  • BLS Certification (Preferred)
  • RN License (Required)
