What you'll Do Supports and carries out the organization’s mission, vision, and values on a consistent basis.
Demonstrates the ability to obtain patient history, perform an age-specific physical, psychosocial and spiritual assessment and initiates appropriate referral, intervention and collaboration between the patient, family, Interdisciplinary team (IDT), physician and partnering facility (as applicable) following policy and practice guidelines.
The RN will identify patient/family goals, develop a Plan of Care with patient, family, IDT, attending physician and/or partnering facility and continually evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager).
The RN will demonstrate the ability to administer, monitor and document medications via all routes using age-specific care practices and provide appropriate patient/family teaching
The RN will identify age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match.

What You'll Need
Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school. Minimum of 1 year of current active nursing experience; with experience in caring for terminally ill patients or those with life threatening illnesses preferred; prefer one (1) year experience in an acute or post acute care setting. An equivalent of education and experience may be considered.
Licensure:
Valid Florida license as a Registered Nurse
Valid Florida State Driver’s License
