RN / LPN - Days & Night

We are seeking a compassionate and detail-oriented Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as a Transition Coordinator/Social Services Coordinator within our skilled nursing facility. In this role, the nurse will support residents and their families through care transitions, ensuring safe, timely discharges and continuity of care.

The Registered Nurses Registered Nurse RN / LPN Licensed Practical Nurse will:

• Direct the day-to-day activities of the nursing assistants
• Registered Nurse Registered Nurse RN / LPN Licensed Practical Nurse complete required record keeping upon resident admission, discharge, and transfer; chart and document per facility policy
• Prepare and administer medications as ordered by physician and perform other drug administration functions per established policies
• Registered Nurse RN make daily rounds of unit and prioritize in a fast-paced environment.
