Status: Exempt | Monday -Friday 8:00 A.M.-4:30 P.M.

Starting Salary: RN/BSN  $35.08 per hour – Full Salary for position $35.08-$47.69 per hour

                                   RN-AD     $33.36 per hour – Full Salary for position $33.36-$49.60 per hour

MINIMUM REQUIREMENTS

• Bachelor’s degree is preferred


• RN MN License


• 1-3 years of related work experience


• Valid driver’s license


• Ability to pass a background check

DUTIES AND ESSENTIAL FUNCTIONS

1. Conduct comprehensive assessments of aging and medically fragile individuals to identify health, safety, and service needs.


2. Develop, implement, and monitor individualized care plans, providing ongoing case management and coordinating services with healthcare providers, families, and community resources.


3. Perform other Public Health nursing duties and related responsibilities as assigned.
