RN Patient Referral Coordinator
Job Description
Purpose & Scope:
The RN Patient Referral Coordinator utilizes clinical nursing knowledge to manage and coordinate patient referrals efficiently, ensuring patients receive timely access to appropriate healthcare services. This role bridges clinical assessment with administrative coordination to facilitate seamless communication between patients, providers, insurance, and healthcare facilities.
Education:
Associate's degree in Nursing is required.
Bachelor's degree in Nursing is preferred.
Experience:
Previous nursing experience in a hospital, clinic, or healthcare setting preferred.
Certification/Licensure:
Licensed as a Registered Nurse with Virginia Department of Health Professions is required, or,
Compact State Registered Nurse License is required
Knowledge, Skills & Abilities
Strong knowledge of medical terminology, clinical workflows, and patient care coordination.
Experience with electronic medical records (EMR), scheduling software and referral tracking systems.
Ability to multitask and prioritize in a fast-paced environment.
Compassionate and patient-focused approach to care coordination.
Ability to manage multiple referrals efficiently in a fast-paced health system environment.
Strong attention to detail and commitment to patient confidentiality.
Self-motivated, self-directed and independent worker
