RN - Registered Nurse Home Health

We’re seeking an experienced Home Health RN to join our Harrisburg team and make a direct impact in the lives of patients and families. In this role, you’ll combine clinical expertise with personalized care, helping patients maintain independence and improve quality of life in the comfort of their own homes.

What You’ll Do

🏡 Deliver Personalized In-Home Care

• Perform comprehensive health assessments in patients’ home environments
• Develop and implement individualized care plans tailored to patient needs
• Monitor patient progress and adjust care plans to achieve optimal outcomes

💊 Provide Skilled Nursing Services

• Administer medications and treatments as prescribed
• Deliver wound care and other skilled interventions
• Ensure compliance with physician orders and care standards

📋 OASIS & Documentation Excellence

• Complete and document OASIS assessments accurately and thoroughly
• Maintain detailed, compliant clinical documentation
• Ensure adherence to state, federal, and agency regulations

🤝 Collaborate & Educate

• Partner with physicians, therapists, and interdisciplinary team members
• Educate patients and families on disease management, prevention, and wellness

What You Bring

• Current Registered Nurse (RN) license in Pennsylvania
• Home Health experience required
• Hands-on experience with OASIS documentation
• Minimum 1 year of acute care experience
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and manage a flexible schedule

Salary: $90K - $107K
