General Summary:

Under the direction of the CCCO and/or VP of Clinical Services, this position is responsible for ensuring State and Federal regulatory compliance in agency and branch policies and practices. This position provides education and tools to ensure the provision of safe and effective patient care.

Essential Functions:

  1. Provides oversight of clinical orientation and education, quality assessment/performance improvement activities, and electronic health record clinical support.
  2. Ensures the development of staff through the implementation of goals, objectives, and improvement plans.
  3. Serves as the key clinical resource consultant and key interface with the home health agency and branch staff.
  4. Develops, disseminates, and educates staff on home health clinical/operational policies and procedures.
  5. Establishes collaborative working relationships across all Support Center cross-functional areas.
  6. Ensures the CHAP/ACHC/State accreditation standards are incorporated into agency/branch processes and patient care-related practices where applicable.
  7. Provides education related to clinical practice standards and other topics related to the provision of patient care, such as improving patient and customer satisfaction.
  8. The policies and procedures related to patient care are in accordance with state and federal regulations.
  9. Develops and maintains clinical programs and resources.
  10. Assists with point-of-care solutions and improving clinical documentation.
  11. Develops, maintains, and provides ongoing review of a comprehensive medical supply formulary.
  12. Develops and ensures 1st in class practices and/or processes to improve OASIS accuracy, diagnostic coding proficiency, and outstanding patient/quality outcomes improvement (inclusive of Home Health Compare, Star ratings, and HHVBP initiatives).
  13. Implement performance improvement initiatives in branch locations to ensure compliance with all KPI metrics and quality measures are being met and achieved.
  14. Document call outcomes, tracks key performance metrics, and ensures accurate data collection in our charting systems and tracking spreadsheets.
  15. Serves as a resource for health care team members to support patient experience initiatives.
  16. Collaborates with clinical and operational teams to relay important patient insights that may impact care or may require interventions.
  17. Assist with conducting regular audits of documentation and call logs to ensure compliance with company standards, EMR practices, and HIPAA guidelines.
  18. Analyze patient feedback and outreach data to identify opportunities for improved service delivery and operational efficiency.
  19. Provides exceptional customer service, addressing patient concerns with empathy and professionalism while escalating appropriately as needed.
  20. 20. Participate in regular training and professional development to stay informed about best practices in patient engagement and home health coordination.
  21. 21. Adheres to company policies, procedures, and compliance standards to maintain the highest patient care and integrity level.

Requirements

Experience:

  • Five years of recent home healthcare experience.
  • Previous management experience required.
  • At least five years of recent direct leadership experience managing day-to-day operations of in healthcare.
  • Expertise in quality/risk management database administration (Midas+ preferred) and an ability to produce timely and accurate reports and presentations is mandatory.
  • Excellent written and verbal communication skills required.
  • Strongly prefer experience with The Joint Commission standards, CMS and/or OSHA regulations & survey preparedness programs. Prefer experience in a multi-facility healthcare system.

Education:

  • A baccalaureate degree in Nursing is preferred.

Licensure/Certification:

  • Registered Nurse, NP or MD

Physical Requirements:

  1. Ability to lift, move, or transport equipment or supplies/materials up to ten (10) pounds without assistance; occasional lifting or transporting of equipment or supplies over ten (10) pounds with the assistance of personnel.
  2. Range of motion and mobility of self to include sitting, standing, walking, bending, stooping, squatting, kneeling, and reaching.
  3. Ability to communicate with patients, families, physicians, co-workers, and visitors to exchange accurate information regarding patient condition and health status. Ability to exchange and express information utilizing language and communicate information effectively.
  4. Ability to hear the nature of sounds. Ability to give full attention to what other people are saying, take time to understand the points being made, ask questions as appropriate, and not interrupt at inappropriate times.
  5. Ability to make independent judgments and decisions. Ability to use logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or problem approaches.
  6. Ability to motivate, develop, and direct people as they work, identifying the best people for the job.
  7. Ability to monitor/assess your and others' performance and make improvements or take corrective action.
  8. Ability to determine financial resources and how money will be spent to achieve quality patient care and account for these expenditures.
  9. Ability to consider the relative costs and benefits of potential actions to choose the most appropriate one.
  10. Ability to collect and analyze numerical, written data, and verbal communication to reach logical conclusions, as well as the ability to determine the time, place, and sequence of operations or actions.
  11. Ability to view the computer screen.
  12. Ability to perform mathematical calculations.
  13. Ability to review, assess, record, or type data quickly and accurately.
  14. Ability to make independent judgments and decisions is required.
  15. Ability to view and operate required equipment.
  16. Ability to determine resources needed to provide quality patient care.
