Job Summary:

We are looking for an experienced and dedicated Registered Nurse (RN) to deliver skilled nursing care to patients in their homes on a per-visit basis. This role works closely with physicians and our clinical team to ensure each client receives care that is both medically sound and personally meaningful.

Key Responsibilities:

• Perform initial assessments and create individualized care plans in coordination with physicians.
• Deliver skilled nursing services in accordance with the established plan of care.
• Monitor and document client progress and adjust care plans as needed.
• Provide education to patients and their families on health conditions and home care techniques.
• Maintain accurate and timely documentation, including clinical notes and OASIS assessments.
• Communicate changes in patient condition to physicians and the care team.
• Coordinate services with other healthcare professionals, such as PT, OT, and social workers.
• Supervise and support LPNs and Home Health Aides as appropriate.
• Participate in case conferences, performance improvement initiatives, and quality reviews.
• Take on-call assignments (as scheduled) for after-hours needs.

Qualifications:

• Graduate of an accredited school of professional nursing.
• Current Arizona State RN license in good standing.
• Minimum 1 year of home health or hospital experience required.
• OASIS and ICD-10 documentation knowledge preferred.
• Strong critical thinking, time management, and communication skills.
• Comfortable working independently and traveling to patient homes.
• Reliable transportation and valid driver's license required.

Compensation & Schedule:

• Pay: $50.00 – $70.00 per hour (based on visit type and experience) + Salary of $1,000 - $1,500 / week
• Schedule: This is a full time role, as a manger and leader you will also be expected to be on call to answer a phone call or text as needed.

Work Setting:

• Client homes (field-based)
• Must be able to lift, stand, bend, and perform physical care tasks
• High exposure risk (direct patient contact)

Reports To: Supervisor / Director
Supervises: LPNs and Home Health Aides (as applicable)
