Job Title: Registered Nurse (RN) - Skilled Nursing Facility (SNF)

**Job Summary:**
The Registered Nurse (RN) at our Skilled Nursing Facility (SNF) is a key member of the healthcare team, responsible for coordinating and providing comprehensive nursing care to residents. The RN plays a crucial role in assessing, planning, implementing, and evaluating the overall well-being of residents, ensuring high-quality and compassionate care.

**Responsibilities:**

1. **Patient Assessment and Care Planning:**
- Conduct thorough assessments of residents' health conditions and develop individualized care plans.
- Collaborate with interdisciplinary teams to establish and update care goals.
- Implement and oversee the execution of care plans, ensuring continuity of care.

2. **Medication Administration:**
- Administer medications and treatments as prescribed by physicians.
- Monitor residents for medication effectiveness and potential side effects.
- Educate residents and their families on medication regimens.

3. **Wound Care and Procedures:**
- Perform wound care, dressing changes, and other nursing procedures as needed.
- Provide specialized care as required, such as catheterization and tube feedings.

4. **Monitoring and Reporting:**
- Monitor residents' vital signs and assess changes in their physical and emotional status.
- Document and communicate observations and interventions to healthcare team members.
- Respond promptly to emergencies, demonstrating proficiency in life-saving techniques.

5. **Patient and Family Education:**
- Educate residents and their families about health conditions, treatment plans, and self-care.
- Foster a supportive environment that promotes resident and family engagement in care.

6. **Collaboration:**
- Collaborate with licensed and unlicensed staff to ensure coordinated and effective care.
- Participate in care conferences, contributing to the development and adjustment of care plans.

7. **Compliance and Quality Assurance:**
- Adhere to all nursing and healthcare standards, policies, and procedures.
- Ensure compliance with state and federal regulations governing nursing practices in SNFs.
- Participate in quality assurance and improvement initiatives.

**Qualifications:**

1. **Education:**
- Graduation from an accredited nursing program with a valid RN license.

2. **Experience:**
- Previous experience in a skilled nursing facility or a similar healthcare setting is preferred.

3. **Skills:**
- Strong clinical and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to lead and work effectively in a collaborative team environment.

4. **Certifications:**
- Current CPR certification.

5. **Licensing:**
- Valid RN license in the state of practice.

6. **Physical Requirements:**
- Ability to lift and move residents as needed.
- Stamina to stand and walk for extended periods.
