RN- Transfer Coordinator Per Diem

Description - External
* 1. Facilitates the movement of patients through the Emergency Department while meeting door to floor metrics.
* Collaborates with the primary nurse to assist with the admission process.
* Collaborates with the ED Provider to facilitate the decision to admit.
* Collaborates with physicians to assess urgency and priority of admission.
* Collaborates with the admitting Physician to assure admission orders reflect the need for appropriate bed placement.
* Collaborates with departments to efficiently complete diagnostics.
* Collaborates with ED leadership to remove barriers for improvement of patient flow.
* 2. Coordinates patient flow through the organization by keeping updated on hospital-wide patient activity.
* Assesses and anticipates bed placement for planned surgical, direct admits and other admissions.
* Uses knowledge and expertise to assure the bed assignment accurately reflects the appropriate level of care for the patient.
* Facilitates internal transfers to meet patient need for change in level of care.
* Proactively manages and problem solves bed placement issues, collaborating with unit leadership or resource nurses.
* Leads bed huddles to strategize patient placement based on dynamic needs.
* Serves as contact for NEOB and local funeral homes during scheduled shifts.
* Monitors future admissions and discharges to assess and determine impact on patient flow, resources and capacity.
* Makes recommendations to initiate capacity planning protocols and provides status of resources and barriers to discharges.
* Role models professional behaviors and accountability for meeting quality measures.
* Actively participates in the development and maintenance of systems to maximize patient flow and optimize bed capacity.
* 3. Coordinates transfers into and out of facility.
* Coordinates transfer requests by evaluating bed availability at receiving facilities.
* Facilitates dialogue with providers to arrange acceptance of patient at receiving facility.
* Ensures compliance with EMTALA documentation requirements by completing concurrent audits on all transfers.
* Conducts daily evaluation of pending transfers and provides updates on bed status.
* Collaborates with Case Management and accepting providers to determine appropriateness of requests for outside transfers.
* 4. Communicates proactively with members of the healthcare team.
* Collaborates and facilitates report of bed and staffing status with on-going and off-going Nursing Supervisors.
* Fosters open communication among colleagues in order to optimize patient flow.
* Promotes positive relationships among units, with external entities and with providers.
Qualifications - External
* Experience Minimum Required
* More than 3 years RN
* Experience Preferred/Desired
* Three to five years RN in acute care hospital setting
* ED or CCU experience
* Education Minimum Required
* RN program completion; BSN or enrollment in BSN
* Education Preferred/Desired
