RN Transplant Coordinator - Urgent Position

Hours: 8:30A to 5P
40 hours a week
Days may vary but would have rotating weekends/holidays
Dress code: Scrubs

Ideal Candidate: Experience in case management, coordination, abdominal transplant, liver diseases, and ambulatory setting. Team player, computer skills, and patient-oriented. Looking for an RN for Liver.

Requirements

Active CA RN License upon submission
Active BLS from AHA upon submission
1 year of experience as an RN in either Transplant Coordination, Stepdown, or ICU, or Case Management
 

Top 3 Required Skills:
Clinical knowledge in either transplant/ICU/stepdown
Organization & coordination
Communication & Interpersonal Skills

Top 3 Preferred Skills:
Transplant/ICU experience/Step down experience
Familiarity with Cerner
Familiarity with technology/excel/etc.

Top 3- 5 Daily Responsibilities:
Seeing patients in clinic with MD/NP support
Reviewing labs and diagnostics and escalating to MD/NP as needed
Ability to perform review of systems and triage patient care needs

Job Description: RN to triage telephonic and electronic messages, do patient education and administer medication.

• Develops and maintains nursing practice standards of care for a specific transplant population.
• Serves as a liaison and provides consultation and education services to support standards of care.
• Identifies and resolves clinical issues relating to the transplant patient.
• Collaborates with staff, the patient, and his/her family concerning patient evaluation and discharge planning.
• Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.
• Coordinates communications with the outpatient transplant population such as transplant clinic visits, community referrals, telephone consultations, outpatient treatments, and subsequent hospital admissions.
• Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
• Participates in collaborative, interdisciplinary research activities related to transplant patients or process improvement activities.
• Performs related responsibilities as required.
• This position may require rotating on call responsibilities as needed, including nights, weekends and holidays.
