Compensation: $75,000 - $100,000 + $4,000 in first-year sign-on bonuses

Daily Responsibilities:

• Conduct in-home assessments and administer skilled nursing care
  
• Educate and empower clients with personalized health instruction
  
• Accurately document patient progress and follow OASIS guidelines
  
• Act as a trusted resource and advocate for patients and families
  

Required:

• MN Registered Nurse license
  
• Strong clinical, organizational, and communication skills
  
• Reliable transportation
