RN/LPN - Case/Care Manager

Make a real impact as a Nurse Care Manager with Care Around the Block. We’re looking for an LPN or RN who combines strong clinical experience with the administrative skills to oversee and coordinate care teams. This flexible, client-focused role is ideal for nurses who thrive on both hands-on care and managing the bigger picture. 

What You’ll Do

• Oversee and support care workers, ensuring high-quality, ethical care for clients


• Conduct comprehensive care assessments (medical, cognitive, emotional, safety, financial/legal)


• Develop, implement, and update individualized care plans


• Coordinate communication between clients, families, and healthcare providers


• Monitor client health, provide crisis intervention, and offer referrals as needed


• Complete accurate documentation within 24 hours


• Participate in on-call rotation and handle additional tasks as assigned

What You Bring

• TN Licensed RN or LPN (Associate’s or higher; BSN preferred)


• At least 3 years’ experience in medical or human services, with knowledge of community resources for older adults


• Proven ability to lead, organize, and communicate with care teams


• Tech-savvy: proficient in Microsoft Office and able to learn new software 


• Reliable transportation, vehicle insurance, and ability to work from home (private, HIPAA-compliant space) 


• Able to pass background, drug, and DMV checks

Full-Time, Part-Time, and Contract positions available. This is an hourly, weekly pay position.

Local travel required.
