What You’ll Do

• Support Clinicians: Assist field staff with care coordination needs and logistical support

• Coordinate Supplies: Place and track patient supply orders

• Care Oversight: Monitor patient care coordination to support strong outcomes  

• Triage & Wellness Calls: Conduct outreach calls to patients and assist with clinical follow-up

• Fax Management: Monitor and route incoming clinical faxes 

• Scheduling Support: Communicate with the Scheduling team to support patient needs

What You Need

• Active California LVN license 

• 1+ year of nursing experience (Home Health preferred)

• Strong Communication Skills: Professional phone and interpersonal etiquette 

• Tech Savvy: Proficient with EMR systems and Apple products

• Highly Organized & Detail-Oriented: Able to prioritize multiple tasks in a high-volume setting

• Team-Oriented: Willing to collaborate with multiple departments

Schedule

• Monday to Friday, Day Shift

• On-site at our San Francisco office (this is not a remote position)
