Job Summary
In fulfilling his or her primary duties and responsibilities, the Sage Glen Nurse Manager should be prepared to provide clinical guidance and leadership to Charge Nurses. He or she will assist the Charge Nurses with the supervision of the CNAs and the overall monitoring of resident care. Administrative skills and effective communication are essential requirements for this position. The Neighborhood Nurse Manager shall exercise professional judgement in carrying out a variety of activities that maximize the well-being and quality of life of residents and promote a culture of inclusion and respect.

Advocacy
Infection prevention
1. Manage and support Canterbury Court’s infection prevention and control program in accordance with applicable federal, state, and local regulations, as well as organizational policies and accepted long-term care practices.
2. Conduct infection surveillance, track trends, monitor outbreaks, and communicate findings and recommendations to nursing leadership and interdisciplinary team members.
3. Assist with implementation and monitoring of infection prevention policies, including hand hygiene, transmission-based precautions, isolation practices, immunization tracking, and outbreak response measures.
4. Conduct and report on routine audits related to adherence to infection control standards.
Clinical
1. Serve as a unit manager by coordinating clinical operations on the assigned unit, monitoring resident status, addressing care concerns, and promoting continuity and quality of care.
2. Manage new admission processes for both temporary and long term residents, including ensuring orders and documentation are uploaded to the chart, assessments are completed, and rooms are in good order.
3. Actively participate in weekly care plan meeting by having an in depth understanding of each resident’s clinical conditions and being prepared to answer resident and family questions within scope of practice.
4. Take responsibility of Charge Nurse’s duties, as needed and directed by DON/ADON.
5. Assist with grievance resolution of from patients and families in collaboration with Social Worker.
6. Ensures that medical supplies, medications, residents’ linen, laundry, and equipment are in good order and well-stocked.
7. Can recognize significant changes in the condition of residents and take necessary action.
8. Have working knowledge of all Residents under Nursing Care on Sage Glen
Nursing leadership
1. Supervise and support nursing staff to ensure residents receive care and services consistent with care plans, physician orders, organization policies, and accepted standards of practice.
2. Hold staff accountable for quality of resident care, timely completion of assigned duties, adherence to infection control practices, and accurate documentation.
3. Work collaboratively with nursing management to identify educational needs and provide training, coaching, and in-service programs related to infection prevention, documentation, resident care practices, and regulatory compliance.
4. Collaborate with nursing leadership team to ensure CPR certification and licenses/certifications are up to date for all healthcare staff. Follow up with individual staff members to confirm compliance with yearly physical.
QUALIFICATIONS
Education & Experience:
· Education: Graduate of accredited School of Nursing and must be licensed by the State of Georgia
· Must be CPR certified
· Experience: Prior work with the geriatric population in a community or long- term care setting is preferred
Knowledge, Skills & Abilities:
· Demonstrated problem solving and conflict resolution skills in issues of staff to staff, staff to resident, staff and family, and resident to resident and between various departments.
· Strong verbal and written communication skills; Computer literate: Word, Excel, Power Point, PCC. Must be able to speak, read and write the English language.
· Thorough understanding of Federal and State rules, regulations, policies, and guidelines governing labor, employee issues related to their job.
· Must have skills in communication, assessment and critical thinking. He or she should be able to work effectively with a variety of disciplines in an individual and team setting.
· Should have knowledge of State and Federal Health Care Regulations, Laws, and procedures
· Strong inter-personal skills; Able to relate well with administrator, directors, managers, staff, and larger community; Able to collaborate across disciplines to integrate and coordinate care management issues.
