Hollywood, FL | Full-Time | Monday–Friday | Office-Based | 9:00 AM – 5:30 PM

We’re looking for a Scheduling & Care Coordination Coordinator to support daily operations for our home care organization serving Medicare and Private Duty clients. who can help keep our daily operations running seamlessly. This role ensures every client has caregiver coverage, schedules are accurate, and communication is handled professionally.

About Us


Ascend Back Office Solutions provides operational support for Access NurseCare and At Home Nurses Care, two trusted home care agencies serving Miami-Dade and Broward counties.


Our teams provide Medicare Home Health, Private Duty, and Care Management services to help individuals receive compassionate care in the comfort of their homes. Behind the scenes, our operations team ensures everything runs smoothly so our caregivers can focus on what matters most: caring for our clients with dignity.


What Success Looks Like in This Role




  • Proactively fill Client open shifts and escalate staffing risks immediately




  • Schedules are accurate, and payroll runs smoothly




  • Ensure communication with caregivers and clients is clear and documented




  • Even during last-minute changes, you remain calm and solution-focused




  • Remain calm and solutions-oriented during last-minute changes




Key Responsibilities - Daily Scheduling & Operations




  • Manage all aspects of the client intake and scheduling process 




  • Ensure all referrals are received accurately and completely




  • Build and maintain positive relationships with clients and referral sources




  • Create and coordinate caregiver schedules based on skills, availability, and client needs




  • Schedule caregivers based on skill, compatibility, and authorization




  • Confirm schedules daily in WellSky




  • Monitor caregiver clock-ins and clock-outs for accuracy




  • Fill open shifts proactively using CareFinder and outreach




  • Manage call-outs and schedule changes professionally




  • Assist with answering and direct incoming calls, ensuring all client and caregiver inquiries are fully resolved (“closing the loop”)




  • Coordinate caregiver schedules across multiple companies and service lines, ensuring accurate assignments based on client needs and availability




  •  Respond promptly to requests and concerns




Documentation & Accuracy




  • Document caregiver and client communication on the same day




  • Review and ensure schedules are accurate and prevent overlapping shifts




  • Maintain clear, detailed notes and schedule updates




  • Finalize weekly schedules




 


Caregiver Communication




  • Notify caregivers of schedule updates promptly




  • Conduct regular caregiver check-ins to support engagement and retention 




  • Maintain professional, clear, and supportive communication at all times 




  • Escalate urgent issues or concerns to leadership as needed




  • Facilitate WellSky training for new caregivers




Reporting & Coordination




  • Submit a weekly activity report 




  • Identify trends in call-outs, attendance, or staffing challenges




  • Assist with holiday and emergency staffing planning




  • Serve as liaison between agency staff, caregivers, and leadership 




  • Assist with Wellsky training for new caregivers




 


On-Call




  • Participate in an on-call rotation as needed (weekly stipend provided when assigned)




 


What We’re Looking For - Required 




  • 2+ years of scheduling experience in healthcare or home care




  • Experience with Medicare and Private Duty home care




  • Experience coordinating multiple caregivers and client schedules




  • Strong organizational, time management, and Technology skills




  • Excellent communication and follow-up skills




  • Ability to work independently and meet deadlines




  • Experience with WellSky, Hireology, and Google Workspace a plus




  • Bilingual (English/Spanish)
