Job Description - School Health Integrated Behavioral Health Provider

Description

Position Summary

AMH is opening a school health center in Graham County Schools this fall. 

The Licensed Integrated Behavioral Health Provider is responsible for providing outpatient mental health resources for patients within the primary care setting. The School Based Integrated Behavioral Health provider will work collaboratively with an interdisciplinary team of providers, including, families, educators, primary care MD’s, NP’s, PA’s, Nurses, clinical pharmacists, medical assistants, and psychiatric providers within the school setting Responsibilities of this role include short term mental health counseling to Appalachian Mountain Health patients to help them build resiliency and internal resources to manage mental health symptoms and cope with stressors in their lives. 

Essential Duties

  • Work collaboratively within a team of medical and mental health providers within a school setting.
  • Maintain open communication and accessibility to encourage mental health integration in the clinic setting.
  • Willing to meet with all patients of all ages, from pre-K, to early adulthood.
  • Training in screening, and clinical interventions specific to children and adolescents.
  • Maintains understanding of Patient and Parental Consent Requirements, and HIPAA, and adheres to best practices for consent to provide care to children, and adolescents.
  • Interviews individuals to assess mental health needs, as well as develops appropriate treatment plans specific to each patient.
  • Performs Bio-psychosocial assessments. 
  • Work with a variety of patients with a wide range of mental health diagnoses, and lived experiences, including substance use disorders. 
  • Provides information and referrals for counseling and service acquisition as appropriate to the assessed need. 
  • Initiates treatment interventions; e.g., individual, family counseling and crisis interventions as appropriate. 
  • Support patients through difficult circumstances with an emphasis on building sense of internal locus of control. 
  • Document all patient interaction and referrals. 
  • Charts and maintains notes within 36 hours in compliance with CMS documentation standards.
  • Protects confidentiality and the abide by HIPPA Laws as well as 42 CFR. 

Requirements

Educational Requirements

  • Master's Degree in Social Work
  • Master’s Degree in Mental Health Counseling
  • 3+ years in a Clinical Setting 
  • Licensed Clinical Social Worker
  • Licensed Clinical Mental Health Counselor
